| Philanthropy
is essential to the patient treatment efforts of Sarasota Memorial
Hospital and other nonprofit healthcare organizations in the
Sarasota area. To accomplish the goals of the Foundation, we need
your caring investment in order to: |
- understand the causes and treatments of complex diseases,
- educate
tomorrow's physicians and nurses,
- espouse
prevention programs for the public health of our children and
grandchildren,
- provide
compassionate and effective care for our patients,
- and
quite simply-save lives

| Ways: |
| Thoughtful Giving |
| The Thoughtful
Giving program provides a way to honor a person or an event-or
memorialize someone you care about and wish to remember in a special
way.
This program is
the perfect way to thank the nurses or nursing units, who have been so
important to you. A donation designated to the Center for Nursing
Excellence through the Healthcare Foundation will support and
encourage their commitment to innovative nursing practices, education
and programs.
The person(s)
honored will receive a letter announcing your special gift in their
honor. If your gift is in memory of a friend or loved one, a card
will be sent to their family. The amount of your gift will not be
mentioned and you will receive an acknowledgement for your personal
record keeping.
Your
thoughtfulness is a gift from the heart to those you care about and a
lasting investment in the future of Sarasota Memorial Healthcare
Foundation, Inc. and Sarasota Memorial Hospital.

| Matching Gifts |
| Many companies
will match the gifts of their current and former employees. When a
donor makes a gift, he/she also needs to include a matching gift form
from their company.

listing companies that participate in a Matching Gifts program, please

| Legacy of Life - Planned Giving |
| Legacy of Life has
been created to recognize and say thank you to those persons who have
made the most personal and thoughtful gift of all; a commitment to
Sarasota Memorial Healthcare Foundation, Inc. through a will, trust
agreement, prepaid life insurance policy, or other estate plan
vehicle.
Anyone may join
Legacy of Life simply by advising the Foundation office that they have
provided for a future gift to the Foundation and then providing the
appropriate documentation. All documentation is confidential.
A careful giving
plan can yield tangible benefits to you and your family, including
current income tax deductions, reduction of capital gains taxes,
estate tax advantages, or a combination of these. In addition you may
receive income for life with certain gift plans.
Legacy of Life
members will be recognized in appropriate publications and specific
recognition areas within the campus of the Sarasota Memorial Health
Care System.
The Foundation
does not provide legal advice. Please consult your attorney or tax
advisor for advice when making a gift of real estate, personal
property, special trust, insurance policy, bequest by will or other
estate plan.

| Corinthian Society-Cumulative Gifts of $5,000 and more |
| The Corinthian
Society is comprised of individuals, corporations and foundations
having made cumulative gifts of $5,000 and more to the Healthcare
Foundation. Corinthian Society members will be recognized for their
support by various donor recognition programs located on the campus of
Sarasota Memorial Health Care System. |
| Samaritans-Annual Giving |
| To meet the needs
and challenges of healthcare today and tomorrow, the demand for annual
giving continually increases. Gifts may be cash, securities, or other
assets. For a complete listing of membership levels, please contact
the Foundation office for a membership package.

| A Donor Bill of
Rights: |
| Philanthropy is
based on voluntary action for the public good. A tradition of giving
and sharing that is primary to the quality of life. To assure that
philanthropy merits the respect and trust of the general public, and
that donors and prospective donors can have full confidence in the
not-for-profit organizations and causes they are asked to support, we
declare that all donors have these rights:
|
- To be informed
of the organization's mission, of the way the organization intends
to use donated resources, and of its capacity to use donations
effectively for their intended purposes.
- To be informed
of the identity of those serving on the organization's governing
board, and to expect the said board to exercise prudent judgment in its
stewardship responsibilities.
- To have access
to the organization's most recent financial statements.
- To be assured
their gifts will be used for the purposes for which they were given.
- To receive
appropriate acknowledgment and recognition.
- To be assured
that information about their donations is handled with respect and
with confidentiality to the extent provided by law.
- To expect that
all relationships with individuals representing organizations of
interest to the donor will be professional in nature.
- To be informed
whether those seeking donations are volunteers, employees of the
organization or hired solicitors.
- To have the
opportunity for their names to be deleted from mailing lists that an
organization may intend to share.
- To feel free to
ask questions when making a donation and to receive prompt, truthful
and forthright answers.

| Gift
Substantiation: |
| Beginning in
January 1994, all contributions of $250 or more need to be
substantiated with a notification from the charity. You are no longer
able to rely on your cancelled check for tax purposes. Your
acknowledgment must also include a statement that says whether or not
goods or services were received in return for your contribution to
take full advantage of this deduction. As of this year, your
acknowledgment must also state the date the gift was received by the
charity.
Quid pro quo
contributions are gifts of $75 or more in which goods or services are
received by the donor, such as; golf tournament entry fees, tickets to
programs and events, and parking, for which the general public would
have to pay full price. It is up to the charity to provide the
estimated fair market value for the goods and services you receive and
notify you on your acknowledgment letter what amount of your gift is
tax deductible.
These are
important issues for all nonprofit organizations, and compliance is
mandatory. If you are making a gift to any charity, make sure that you
receive the proper documentation and keep these for your records.

| The
Healthcare Foundation respects each donor's right to privacy. Your
information will not be rented or sold to, or, exchanged or
shared with any individual, organization or corporation for any reason
except as required to conduct the Healthcare Foundation's business. |
