At Sarasota Memorial Nursing & Rehabilitation Center, we offer short-term rehabilitation, as well as
extended care and provide a comprehensive care program that focuses
on you and your family. Our facility offers all the comforts of home
combined with skilled care for your individual needs.
Whether you are here for a short stay or to make a new home here,
we will develop a program that best suits you. As your needs change,
we will adjust your care plan accordingly.
 Family and Resident Involvement
We believe that you and your family members should
be actively involved in planning your medical care and treatment.
We encourage you and your family to participate in the development
of your care plan through regular resident conferences. We also schedule
follow-up conferences to keep family members informed of your progress.
When it is time for you to return home, we can help during the transition
by providing a home assessment. If indicated, the rehabilitation
team will evaluate your home and recommend adaptive equipment and
physical modifications to make living at home easier. We also can
provide referrals to community services to ensure your at-home needs
are served.
Care Team
Our supportive, caring staff is on duty 24 hours-a-day to assist
you. Our fully licensed and thoroughly trained team have extensive
experience in rehabilitation and geriatric care. Once you become
a resident, our care team evaluates your rehabilitation
needs and develops a care program that best suits you. The following

Admissions
Our admissions program includes a pre-admission nursing assessment
to determine your care needs. Our business office staff will assist
you with financial forms and answer any questions that you may have
regarding insurance and payment plans. Admissions are made seven
