The pre-registration process will assist us in preparing for your upcoming hospital visit. Pre-registration is not required, but doing so will expedite your check-in by reducing questions during the registration process.
Please complete pre-registration within three (3) full business days prior to scheduled service. If you find you have questions or need additional assistance with the pre-registration process, please call our Pre-registration Department at (941) 917-6775.

Do I need a doctor to receive medical services?
Yes, a physician order is required to receive most medical services with the exception of Emergency Room, Urgent Care or Walk-In Medical Services. If you need a physician, our HealthLine Physician Referral Service is available to assist you with a referral, as well as help with making an appointment. Contact the HealthLine online at healthline@smh.com or call at (941) 917-7777 between 8:00 a.m.-5:00 p.m. for this free and confidential service.
How do I know if my insurance plan will cover the medical service(s) I require?Sarasota Memorial has contracts with most major medical insurance companies. Patients should verify coverage with their insurance company. See list of Health Insurances Accepted at Sarasota Memorial. Co-payments are due at time of service.
Who will see my personal information submitted online?
Your protected personal health information is accessed on a need-to-know basis by Sarasota Memorial registration personnel in order to make your admission and stay as convenient and expedient as possible. Our Notice of Privacy Practices provides legal assurance and pertinent information regarding patient rights and confidentiality of the use of your health information. You will also receive a copy of this brochure when you check-in at Sarasota Memorial.
What should I bring on the day of my visit?
Please be prepared with your physician order, insurance card(s), driver license and/or photo identification. Co-payment is required at this time. Scheduled inpatients are advised to bring a copy of their Advanced Directives or Living Will for hospital records. Please leave any personal valuable items at home.
