Rehab Services: Designing A Care Plan

Getting Well: Designing a Care Plan

Our rehab team evaluates every patient’s strengths and abilities , then develops a unique, personalized plan to restore physical independence. Our care plans address everything from walking and balance therapy to leisure skills and self-care, as well as meal planning, driving and social and financial services.

    Every patient has a case manager assigned to them who coordinates care and services from admission through discharge. They act as a personal liaison helping patients and families through the CRU experience, coordinated care and discharge planning.
